private Pay & eap clients

Private pay clients, and those referred through Lyra and CCA/Headspace would begin enrollment directly on this site. (Headway, the EMHR portal is not designed for this purpose for you as it is only for insurance based clients so please don't try there it will be a waste of your time).

These are the required steps required to book your session and should take about 10 minutes and then you can book your session. There is currently no wait list and you are likely able to schedule within 24-48 hours and possibly same day.

1. Read HIPPA rules

2. Read Psychotherapy Agreement, click button below

3. Read Cancellation/Financial Policy, click button below

4. Read Consent Form, click button below

5. Fill out Intake Form & Sign, 

- on the intake form, two of the questions are only for Lyra referred EAP clients, others please write n/a, 

- by signing, it means you have read, understand and agree to the psychotherapy agreement, cancellation/financial policy and consent to therapy

6. Proceed to schedule your aptmt through the Booking page 

 

*  LYRA clients:  you always will schedule through Lyra, please complete your intake at least 24 hours before your session to give ample time to verify your authorization, also we will need to set up a credit card authorization on file  via Square in case of no show/missed aptmts as Lyra often does not cover them

*  CCA/Headspace:  you are authorized to have  45 minute sessions and you can proceed to the booking page after completing the intake form
